Complementary (Co)Care
Most families don’t come to us because they were already planning to hire a midwife.
They come because they’re pregnant in Puerto Rico—or elsewhere in the Caribbean—and quickly realize that the experience of navigating pregnancy and birth feels harder and more lonely than what they expected! The system works differently here—it’s not always what you’re used to, especially if you’re coming from the continental U.S. Communication can feel unclear–it can be almost impossible to even get a doctor’s office to answer your phone call…and when you’re trying to make important decisions about your care, your birth, and your baby, that can feel overwhelming.
CoCare was created for families who already have an OB and will be giving birth with them, but want the deeply personalized care, guidance, and holistic support that standard maternity systems often overlook. Through CoCare, you’ll receive one-on-one prenatal, labor, and postpartum support (depending on the package you choose) from Anjelica Malone and her team. Anjelica holds a Master of Science in Midwifery and is an International Board Certified Lactation Consultant (IBCLC).
I don’t even know where to begin with how grateful I am that Anjelica was by my side as my midwife. From the very beginning of my pregnancy to months after my babies were born, she supported me through every single stage. She provided amazing resources, and anytime I had a question or concern, she helped me find the answer or shared her deep knowledge with me. The childbirth class she gave my husband and me truly helped us understand what to expect during labor, birth, and postpartum. She even took the time to sit down and help me meal-prep, planning out exactly what would be most nutritious for my recovery.
Anjelica really listened to me, making me feel completely confident in myself and my ability to have the natural birth I wanted. During labor, she and my husband were my absolute rocks; I truly don’t believe my birth experience would have been as wonderful as it was without her there. Through the hardest and scariest moments, she reminded me of what I was capable of, keeping me centered and grounded. Throughout this whole journey, I felt profoundly supported. I cannot thank her enough for her love, care, and guidance.
Anjelica began supporting families in Puerto Rico in 2013. She and her team understand how care actually functions on the ground—not just how it’s supposed to work, but how it really works day to day.
Our role is to bridge that gap.

- For others, it means thoughtfully planning a transition back to the U.S. for birth—while still navigating pregnancy care locally in the meantime.
Either way, you’re not left guessing. We break everything down into clear, manageable steps so you know what to expect, what your options are, and how to move forward at each stage of pregnancy and birth.
We also support your partner so they’re not walking into this experience unsure or overwhelmed. They understand what’s happening, how to support you, and how to stay present and advocate for you when it matters most. This isn’t about pushing a specific type of birth or philosophy.
It’s about helping you have a well-supported, informed, and steady experience—no matter how your birth unfolds.

CoCare offers you the option of having a midwife and an OB (who remains your primary medical provider and will deliver your baby) supporting you on your pregnancy journey. This is a key part of the SUNDIAL wraparound care model. It may seem redundant to see both a midwife and an OB, but the unique care and knowledge of a midwife can make a world of difference in a mother’s probability of having a vaginal delivery and returning to her pre-pregnancy healthy state without complications.
At each visit we will discuss different topics to help you prepare for your ideal birth, as well as understand the complexities of postpartum recovery. Anjelica provides recommendations, referrals, and advice based on our unique locale. She also helps clients coordinate plans for giving birth in the continental United States if that is their preferred option.
Visits cover:
- What’s normal and what’s not–when to seek help.
- Nutrition and how to maintain a healthy weight in pregnancy
- Supplements and herbal safety
- Childbirth education (either a limited prep or full 2-part series)
- Movement and exercise recommendations – how to remain active and safely exercise.
- How to improve your chances of having a vaginal birth; including optimal fetal positioning for labor, appropriate vs. inappropriate uses of induction, and the benefits and risks of different pain management options in labor.
- Recommendations for relief for common pregnancy discomforts
- Evidence and education about gestational diabetes testing, ultrasounds, lab work, group b strep, newborn medications and shots, labor interventions, and avoiding tearing during birth.
- Planning for postpartum- recovery supplies, nutrition, normal vs. abnormal signs in mother and baby, mental health, and when to seek help.
- Weight loss
- Breastfeeding education and newborn care
